APPLICATION PROCESS

Grant applications should be submitted by the department chair, division chief, or fellowship director of the academic institutions and must be received by Allergan Medical Affairs no later than September 28, 2007.

Application components
The application must include:

  1. Information about the fellow’s institution and fellowship program
  2. How the funds will be used to advance research in glaucoma or corneal or retinal disease
  3. A summary of the research program and educational plan for the fellow’s research

More than one application per academic institution may be submitted, provided that each application is from a different department; however, only one Horizon Grant will be granted per institution in a two-year time frame. Applicants cannot have a familial relationship with a member of the Academic Advisory Board.

SELECTION PROCESS

Grant applications will be reviewed by the Allergan grant review committee with the assistance of the Horizon Academic Advisory Board, an independent advisory board made up of experts in the fields of cornea, glaucoma, and retina.* Applications will be reviewed and grants will be awarded based on the strength of the fellowship program, the clinical and research plan for the institution’s fellow, how the grant funds will be used, and activities the fellow will be engaged in while in the program. Grants will only be awarded to fellowship programs that conduct clinical research in the diagnosis or pharmacological treatment of glaucoma and corneal and retinal diseases.

The Horizon Academic Advisors board is composed of two advisors each in cornea, glaucoma, and retina. Three of the advisors will serve one-year terms; the other three will serve two-year terms. Horizon Academic Advisory Board members do not participate in the evaluation of applications from their own institutions. Horizon Academic Advisory Board members serve in their individual capacities and any opinions/views expressed in connection with their service may not reflect the opinions/views of their employers or other affiliated institutions. Views expressed by board members are not sanctioned or endorsed by their employers or affiliated institutions.

*If appropriate applications are not received in a specific disease category, the grant money will not be awarded.

PROGRAM ADMINISTRATION/STIPULATIONS

Allergan Horizon Grants are educational grants paid to the institution. The grants will be distributed to the institution in January 2008. The institution is responsible for administering the funds in accordance with its prevailing procedures and policies, and the stated stipulations of this Allergan grant.

Use of funding: This Allergan grant is intended primarily to support salary and research of the institutions’ fellows and shall not be used for overhead, indirect, or fringe costs. Use of award funds toward direct expenses necessary for the proper operation of the fellowship program (eg, salary of a technical associate, purchase of relevant laboratory equipment, etc) is also acceptable.

Annual progress reports: Representatives of the institutions receiving an Allergan Horizon Grant will be invited to Allergan in Irvine, California, for the Allergan Horizon Grant Awardees Forum to present A Year in Review of their fellowship program. This will take place in June 2008 to report on the 2007–2008 academic year.

Transferability, forfeiture, and termination: Should the institution anticipate changes in their fellow’s program circumstances or situation for reasons including, but not limited to, discontinuation of the program, Allergan must be notified promptly. If forfeiture, temporary suspension, or termination of the grant is desired or warranted by Allergan or the institution, a written agreement of the same shall be executed, terminating the agreement. Any unused grant funds must be returned to Allergan.
